Proof of ID

You will be offered a health check via telephone or video consultation with the healthcare assistant. This check is very important as the practice will then have some information about you.

If you are on regular medication please attach a copy of your medication along with the form and inform the HCA during the consultation.

If you have children who are under 5 years of age they do not need a health check but it is very important that you attach a copy of the immunisations from their red book.

You will be registered with the practice rather than an individual doctor and therefore free to see any doctor.

Please allow two working days for your request to be processed,(3 working days if ordered via the pharmacy) the prescription can be collected either by yourself or your nominated pharmacy

prescriptions can also be sent electronically to a pharmacy of your choice. This process is known as electronic prescribing, you will need to speak to your local pharmacy to set this up for you.

The practice policy is that all patients who are taking medication long-term basis are reviewed.

If you have been seen in a hospital clinic and have been advised to see the GP for a medication please allow up to 4 weeks for the letter to arrive at the surgery and be processed.

HOME VISITS

If you need a home visit for emergency medical reasons please call before 10 AM if possible.
Out of surgery hours your request will be dealt with via NHS direct.

Please note

  • Routine appointments are for 10 minutes.
  • Please arrive ontime for your appointment time to ensure you do not lose your place
  • If you are late by more than 10 minutes you may have to rebook the appointment.
  • Only one person may be seen per appointment.
  • Please ask for a double appointment if you have more than one problem to discuss or if you think your issue may take some time.
  • Please ask reception to book you a translator if you need help with interpreting.

A carer is anyone ( this includes children and adults) who looks after a family member, partner or friend who can not cope alone due to illness, disability, a mental health problem or an addiction.

Community Treatment Team

The community treatment team (CTT) works with adults in the community with an acute physical need who could potentially be treated at home, rather than attend accident and emergency (A&E). The aim of the service is to prevent unnecessary hospital admissions for those people who are experiencing an acute, physical health crisis, for example: exacerbation of infections or worsening of long term conditions such as COPD and falls without injuries.

Care Navigation

Care navigation is a way by which you  can get your matter dealt with quickly by the most appropriate service. This signposting service can be used in many ways eg instead of calling the surgery to make an  appointment to see your GP for an antenatal referral our reception staff  will signpost you to the online self referral antenatal booking form.

This will ensure that the referral is done in a timely manner without having to wait for an appointment to see the GP.

When you phone for an appointment to see the GP , the receptionist will ask you details of why you want to see the GP, this is so that the receptionist / care navigator can then assist you in making an appointment with the most appropriate person/service.

You do not have to give any details to the reception staff if you would prefer not to however if you do, it will help the staff to signpost you to the most appropriate service to meet your needs.

The choice to accept any service that you are signposted to is yours, you will not be denied an appointment with the GP.

Non NHS Services

The NHS provides most health care to most people free of charge, however there are exceptions for
example prescription charges.

There are also a number of services which are not covered by the NHS and so a fee is charged for
these.

“ All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.

 The average pay for GPs working in the Ilford Medical Centre  in the last financial year was £93,603 before tax and National Insurance. This is for 0 full time GPs, 8 part time GP and 1 locum GP who worked in the practice for more than six months.

 However it should be noted that the prescribed method for calculating earnings is potentially misleading because it takes no account of how much time doctors spend working in the practice, and should not be used to form any judgement about GP earnings, nor to make any comparison with any other practice.”

I have a form that needs a gp’s input, how can I get this arranged?

The reception staff will then advise you further with regards to any fees payable and the time taken
to process the documentation.

I only need a gp’s signature whats the problem

When a GP signs a certificate or completes a report it is a condition of remaining on the medical register that they only sign what they know to be true. Therefore in order to complete even the most simplest of forms the GP must check the patients medical record.

Why not write what I ask

GP’s are under the BMA guidance and are tied to strict protocols of integrity, accuracy and upholding professional standards. Therefore they can only write what is factually correct and evidenced by their own medical assessment or from another health professional in writing.
Some patients can be disappointed by what they may want in a letter has not been reflected. Putting excessive pressure on the GP on something that Is not factually accurate is regarded as a breach of the Zero Tolerance Policy.

Why do I need to pay upfront

In the past we have completed reports and forms before payment has been made , but sadly there have been many occasions when the GP has completed there work but the payment has not been made. Therefore the fee must be paid in advance at the time of the request for their letter/form is made.
